Performance work status

Hi All,

I think I mentioned a few weeks back that the next major area for
improvement would be the messaging engine code. Since then I've found some
quite neat optimisations and now have the perf1 score on my DX/4-100 system
down to 33.5 microseconds - in addition general messaging operations
(connects, disconnects, paging, etc) should also be faster.

I've also tinkered with an idea that I seem to remember Joerg Wittenberger
suggested of having a msg_reply_receive() function to avoid the transition
into kernel space twice - with just a simple version that does this by
calling msg_reply() and msg_receive() in turn we get about 1.5 us of saving
- I'll play around with this some more (expanding out the code) as it may
save us some vital usecs.
